The Patient Care Coordinator is responsible for outreaching patients and assessing their physical, social and psychological needs. In this role the coordinators will document patients medication height, weight, glucose readings and vital signs as well as any changes in the patients medical history and mental status. The Patient Care Coordinators acts as a liaison between the patient and physician to communicate the patients history and current needs in order to provide appropriate and effective care. Additional duties in this role includes scheduling patient appointments, insurance verification, obtain referrals/pre-authorizations, ordering of medical equipment and educating patients/families on device utilization.
Qualifications:
- Associates degree or equivalent experience
- 2+ years experience in healthcare/medical office with customer service
- 2+ years telephonic call center experience
- Advanced knowledge of Microsoft applications (Excel, Word, Outlook)
- Excellent written, oral and communication skills
- Exceptional attention to detail, accuracy and discretion
- Must be able to perform all essential job functions without accommodation
- Must now a computer and have high speed internet
- Must have a smart phone compatible with Nextiva app
- A designated quiet workspace free from distraction
- Previous experience working remote
